So if you have 20% copay on that medicine your copay's gonna be $200. Now later perhaps six months later the drug maker will pay a $500 rebate back to the insurance company so

But employers who are ultimately responsible for setting those contracts they like the rebates because it allows them to offset other costs in their administration of their health benefit so they don't often engage in the contract with a PBMs where the rebate would go back to the patient that's the problem.
