The problems were attributed to squirrels chewing through the insulation of the outdoor coaxial cable, a physical kink in the line, and the use of outdated RG59 grade cabling instead of the required RG6. Additionally, the signal was found to be "too hot," requiring the installation of an attenuator to prevent feedback loops.
